RELEASE OF LIABILITY - MEDICAL AUTHORIZATIONS - EMERGENCY CONTACT INFORMATION


I/We, the parent(s) or guardian(s) of the baseball player named above give my/our consent for him/her to participate in any and all team activities. I/We understand that baseball is a contact, team sport which will expose my/our child to injuries during play. There are many sources of danger in baseball including but not limited to pitched, thrown and/or batted balls, swinging and/or flying bats, collisions during fielding and/or base running, other injuries during fielding and/or base running and other hazards. I/We understand these dangers and hazards to be part of the game of baseball that may injure my/our child. I/We accept all dangers, risks and hazards incidental to my/our child's participation, including transportation to and from activities, and I/We do hereby waive, release, absolve, indemnify, and agree to hold harmless Anderson Bat Company, its organizers, members, sponsors, directors, officers, supervisors, employees, agents, representatives, participants, and persons transporting my/our child to and from activities, for any claim, damages, injury, action, and/or litigation arising out of an injury to my/our child, whether injury is the result of negligence or any other cause. Warning: Protective equipment cannot prevent all injuries a player might receive while participating in baseball or softball. A-Game Academy does not limit participation in its activities on the basis of disability, race, color, creed, national origin, gender, sexual preference, or religious preference.
